We would like to have a form created on a microsoft from or powerapp application that is connected to an sharepoint database. The goal would be to create a check in check-out form for our starters. They will fill in their name and email address and at that point, the information should be fetched based on those two unique identifiers. Afterwards when they want to change anything they can go into the form again and fill in these credentials where the form will populate that information from the database. We currently use Sharepoint to manage customers projects. Every customer can login and have a personalised dashboard which pulls data from excel spreadsheets to generate the customer dashboard, there are also links to a central library of PDF's that the customer can pull up from the dashboard for reference.
