B2B Lead Generation/Hot-Key's

I am looking for somebody to Generate B2B energy renewal leads in England ONLY. We require the following information for a lead to be valid: Decision Maker Name Decision Maker Contact No. Business Name Business Address (Incl Postcode) Current Energy Supplier Current Estimated Annual Usage OR Copy of Recent bill Current Contract end date (within the next 6 Months) - (ideally 3 months or less) Optional (most likely to convert) Callback Date/Time for when we can call back and close the deal. We are willing to pay per lead however we would like to see a conversion from the leads so an initial payment will be made for the lead. Ideally we would pay an upfront fee for the leads & any that are converted would warrant an extra payment on top of the initial amount to encourage the lead generator to send legitimate leads to us. The leads can come through to us as either emails or hot-keys. In any proposal please detail how many leads would be generated from our chosen budget & also how long you would expect to take to fulfil the request.
