Get Html Form to submit using Sendmail

Hello, I have an html page and want to. 1. get the form to submit using a sendmail php function 2. forward to either a confirmation page -OR- error page. All pages have been created already. -The webhost is 1and1. -I will give you ftp access to complete this project. -The html was generated using prototype creation software, so it is very 'templatey' but still functional html. -This html file should forward the form information to a php file (which will send). You'll need to create or provide that file. I'm posting so that you can view for your reference the relevant form fields. If you would feel comfortable with this project.
