Create a form in Laravel framework (deadline 28.03)

From scratch Framework: Laravel Written for PHP Version 7.4+ Task: Create a similar form like: [login to view URL] Design (similar) but responsive as it need to be loaded as an Iframe in a wordpress page Functions: 1: On the left side should be a dropdown menu with prefilled template (letters) where the user can choose from 2: When user fill in his user details on the left side they are shown realtime on the rightside of the loaded template (letter) 3: User should sign the letter digitally (make a signature) 4: Once that is done user can download and save the letter as a PDF or Docx (letters will not be saved on server) 5: Option in backend to add more variables that can be placed in the letter (for example an attribute phonenumber) 6: recapatcha v2 should be implemented to prevent spambots while clicking on "generate PDF" Example in clip (gif) Maximum budget $ 50,- Milestone prefunded will be released once the developer has showed that it works and installed on the webhosting.
