Wordpress Plugin Extending the Comment System Functionality

I need a wordpress plugin that extends the comments functionality to include a 5 star rating system. The comments will include a box above the comments form that allows a 5 star review to be made about the posts and custom post types (wherever there is a comment form). The rating system must have 6 total 5 star ratings. The top 5 star rating will be an overall average of the other 5 ratings. So the rating system will look like below: Overall ***** Compassion ***** Quality Care ***** Goals Met ***** Timeliness ***** Communication ***** --- Comment form here ---- I dont need it to look great as wen handle the CSS and styling. Please do not give me quote if you cannot work on this right away. I need this done in the next few days.
