Contact form php script - layout needed too

This project will be used as a guide for selecting coders I shall work with in the future, hence multiple winners will be chosen. For that reason the script is a simple contact form with a twist that allows its global use. The ideal coder should: * Be familiar with the following: - [login to view URL] integration - [login to view URL] integration * Be able to create html templates based on elegant css styles. * Write the code for the script from scratch in php (mysql if needed) and never use the code again to any other projects (full ownership should be delivered) * Be available during weekends too, so if you work for a company that is closed on Saturdays/Sundays please do not bind. * Be fast, accurate and able to deliver a bug free script. Detailed specs and mockup will be delivered to the selected coders, hence there is no need to bid for now. Please do not add any canned introductions - I can see your portfolio anyway. Just verify why you feel you meet the criteria above and also define your hourly rate in USD. Thanks!
