9103 Model Applications

I am a photographer that is trying to make it easier for models to post interest in modeling for me. I need a script that will allow prospective models to post photos, personal information and interests into a form that will be viewable only by myself and those I choose to allow access. The form would need to include the following: 1. Personal information about the model... Name, Age, Location, Height, Measurements, etc 2. The ability to upload up to 10 photos. 3. A list of interests that the model can check off individually. This section needs to be editable by me to include as many or as few items of interest as are appropriate. 4. Schedule of availability. After submitting their information on the form, I need the model to be able to sign in to update her profile. The results of all profiles should be in a list viewable only by me and those with the password. If you have any other questions about the project, feel free to ask.
