Website Info Form (PHP) 1 or 2 page

Simple project for experienced form builders. I am linking to a sample form to be very clear about what we need. Link: [login to view URL] The final form should be very similar to the form in the Link above, only with some additional information requested in the form. PLEASE TAKE NOTE of how "hidden" lines and boxes appear when certain responses are made. For example, when the "add spouse" button is pushed, a new line appears where spouse info is entered. Also, note how additional info areas appear when any of the "yes" boxes in the second section are checked. It is important that this form appear to be very small until people respond to certain areas, when new info boxes then appear. IF YOU ARE UNFAMILIAR WITH THIS TECHNIQUE, PLEASE DO NOT BID. The info itself can be posted in any manner you are familiar with, so long as it is available with our servers. Currently, our form uses a rather simple CGI script for processing, but we are open minded to other means by which to process the info--but it MUST be tied to an email address submitted by the user. PLEASE SPECIFY YOUR PREFERRED METHOD OF FORM INFO DELIVERY IN YOUR BID. FYI, the servers we use are equipped with MY SQL if that is important to you, but this should be a relatively simply info form. This is the first of several projects we have pending for the right programmer. Payment by Escrow only, upon satisfactory completion of the form.
