troubleshoot javascript conflict and custom form fields added to Joomla 3.1

I need 2 things done and I need someone who knows Joomla 3.1. I use Yootheme Nano 3 template with Warp 7 framework, so there is a particular way to add customisations to scripts, if needed. I do not what changes that will be overwritten or will break other 1. I would like an integrated Joomla form with custom fields for taking reflexology bookings. I currently have a modal form and you can see the fields that I need on the form at . It is a modal form, but I am looking for a form that I can add from Joomla contacts, and that keeps with the style of the contact form that is currently there . 2. I have a javascript conflict. I know the Javascript file that is causing the problem, and need a solution that fits better with the framework that I am using. Basically a javascript file for a reflexology foot chart stops a Yootheme widgetkit google map from displaying. On this page you will see that there is a google map module that is currently not displaying the map because the reflexology foot chart script is being called. Google map module page: Reflexology Footchart page: On this page there is a reflexology foot chart, that highlights parts of the feet when the mouse curser hovers over them. A previous programer included all the javascripts, including the javascript library in one file, because he had problems calling the library when it was needed. This was an old solution, but it is not suitable for the Warp 7 framework I am now working with.
