Ajax/javascript Guru needed

I have a life insurance website build in wordpress platform, I need to create a quotation form for users to submit their information. I Need an Ajax/javascript guru to build this form, the design was created by me, I can provide you with a psd file before you start working. The process should be as follows: Users need to submit their zip codes, when they enter it, the system will check if it's a valid us zip code or not, if not, the system should display a warning that this zip code is not a valid US zip code, when the user enters a good zip code, the system will open a popup ajax/javascript form designed by me. On this form, the system will automatically display the state associated with this zip code and it will also display the zip code entered, also the user will have multiple fields to enter, those fields should be dynamic and the admin can have a full access to add, delete and modify each field. Also a validation form should be set on all fields (required), a us phone validation should be integrated with the phone field. You need to have a good experience with such forms. I need an expert person to do this job. You need to send me some ajax/javascript jobs you did before.
