86958 Flash/PHP form issue

[login to view URL] is a website that I designed for a client. I used the page-flip flash script at [login to view URL] to create the page flip motion. The final page, under the "contact" tab is supposed to be a contact form (I took it down for the time being until it actually works). The submit button has an actionscript that is supposed to load the php script to email the information. I'm not sure if it is a Flash issue or a PHP issue (I'm a newbie at PHP) and I'm more of a graphic designer than a programmer, so I need help to correct the form.
