Downlaod the PPT scope - I want you to create an PDF (version 7 compatible) form that a user can enter details into and email to myself. I have made an image and refered to the types of dropdown options. Look at the notes in the PPT document. Drop Down 1 options: UK Canada USA South America Middle East Asia Dropdown 2: Under 5 Seconds Under 10 Second Under 20 Second Under 40 seconds Greater then 1 minutes Greater then 3 minutes Dropdown 3: Yes, I have to scroll a lot Yes, but the page fits okay No scroll bars Dropdown 4: 800 X 600 1024 X 768 1280 X 720 1280 X 768 1280 X 800 High resolution Dropdown 5 Yes It wasn't a smooth animation It didn't transition at all Dropdown 6: Did not play at all. Took a few seconds to load Played instantly and smoothly. Dropdown 7: Did not play at all. Took a few seconds to load Played instantly and smoothly. Please make all entries (except comments) mandatory. Also add a Submit button, this will use the Acrobat Mailto: funtion to email the content to my email(attached in the PPT) So when a user clicks the button, it opens and email and attaches an xml file with the users comments – the email address should be populated with my email and the subject should be “E-Learning Review” Please clean up might alignment and spacing, make it look professional.
