DreamMaker Software Modifications- Job open to rmijares only

This project is open to rmijares only (all other Service Providers need not bid). The project entails creating a v2.0 version of the Dream Maker software that you previously created for me, with the following modifications: 1) Change the name from "Dream Maker v1.0.0" to "Brain Control Wizard v2.0". 2) I will supply completely rewritten default statements to be displayed in "ticker tape" fashion (they will all be questions instead of statements). 3) I will supply my own pre-written text for you to add to the ReadMe file. 4) I'd like to change the Dream Maker icon from the present stylized graphic of a man ==> to a picture of a brain (see 2 attached royalty-free reference pictures of a brain); and change the color of the header bar from blue to yellow (see shade of yellow used in [login to view URL] header. 5) I'd like the scrolling "ticker tape" area to have a dark blue background with yellow letters. After installing Brain Control Wizard, before launching, a screen should come up requiring the program user to "register" it by providing their first name and primary e-mail address. I've generated the code for the form below, which includes the destination of the captured info. (I've also attached the code in Notepad in case the following code gets messed up.) Please feel free to beautify the code to suit your design of the Brain Control Wizard panel.
