HTML5 Web App (NOT iOS APP)

Developer sought to build a mobile-friendly web app which uses HTML5 or CLASSIC ASP and VB/VBScript to gather patient details in an emergency situation, and inputs it into a Microsoft Access database on a remote server. Ability to store and upload when offline highly desirable. This is the second time we have attempted this project - our last developer completely missed the brief and developed an iOS only app which we cannot list on the App Store. The Access database has already been built and is available to review. We envision the flow of the app as follows: Login Page - match username and password from database. Form 1 - Input Patient Details, check against database to prevent duplicates. If no duplicate, move on to Form 2. If duplicate, give user the option to use the existing PatientID (ie, second incident with the same patient) or create a new one. Form 2 - Input Incident Details. Users can then SKIP/FINALISE if no other pages need to be completed, or add Vital Signs & Events using Form 3 & 4. Includes a Geolocation field. Form 3 - Vital Signs. User can add many sets of Vital Signs to one incident. Form 4 - Events. Users can add multiple events to the Incident, and add detailed comments to each event. Includes a camera/image field. Form 5 - Once complete, user can view summary to check patient details and incident report number (primary key from database), and finalise, or go back to edit.
