"Tell your story" form script /site

I will explain what I need from user and admin perspective. The script needed does the following. USER: 1) User goes to [login to view URL] and finds the following form he/she needs to fill in: Name of story: Short description of story: Write your story here: Then he/she needs to: Write their birth year: Select their country from a "roll down" menu: Select their sex: Write name/nickname: Optional things the user can do: Upload pictures to go with the story. User clicks "submit" 2) User is presented with a page where it says either: 2a)"Your story was submitted. Wait for approval" OR 2b)"Your story was submitted. It's online here!(link)" When the story is published it should have the following link name/structure: [login to view URL] The link of the page of the story need to have the same name as the title of the user's story as selected by user in step 1. If identical names the story get the name "nameofstory02.php" and so on. 3) On the story page [login to view URL] I need there to be a comment field at the bottom of the page at the end of the story where surfers can give feedback. I also need it to be a ranking script from 1-5 where people can rate the story. ADMIN: 1) It must be possible to toggle on/off if I want stories to be published instantly, or if they need to be reviewed before they are published. 2) Is this possible: In the comments section there should be something that makes it impossible to post URLs in order to avoid spam. All http:// and or www. and or .com .net .org should be filtered away. 3) On [login to view URL] I want the latest stories to appear as they are published. Like a live updated list. The last submitted story is first and so on. 4) On [login to view URL] I want the possibility to have a "Top list" of links to the stories that have received the best and worst ranking from the ranking system. I should be able to select how many top submissions to display. 5) Search engine friendly URLS. I want the [login to view URL] page to give search engine friendly URLs to the content. A new story is submitted: [login to view URL], not [login to view URL] and so on.. 6) Easy to change templates. If I need to change the background color of the story pages, add some advertising etc. it can be done by editing one template. 7) Installation instructions needed so that I can get another guy than you to install it. Script will not be used on microsoft servers. However, you will need to install it on my hosting for the first time. 8) Final script may be used on more than 1 site. 9) After the script is installed I may need help to tweak a few small things. You need to be available for this. PAYMENT ON COMPLETION: Payment after full working demo (on your site), installation on my site and 5 days of me test-running it. Will do Escrow.
