JavaScript form to calculate fee and generate UPS label

Need website javascript solution where the customer fills the following information: 1. how you would like to submit the application ( option 1 - postal or Walkin) 1. if the customer selects Postal then the system will ask the applicant to fill the following information 1. First Name, Last name 2. Building or street address 3. apt or house number 4. city name 4. zip code, city, and country. after completing this - the customer will be asked to validate the address using UPS API. UPS API will validate the address and generate Label and show total fee to be paid by the customer. if the customer selects - walk-in then customer gets email notification with template.
