Patient Database and Dictation

Problem: Medical information is the burden of all medical clinics. I am part of a larger clinic that uses a large database for billing but not generating medical notes, records. I am currently using a program with voice recognition (using Dragon as the voice engine) but it has a lot of wasted fields and limited in handling the information that is the best for me. All it really is is a database running on MSSQL Server on my computer. It can not be modified. Goal: Create a database that will maintain patient data including demographics, history and physical exam details. Key component is the ability to create and save patient reports and clinic notes. This database should work as a stand-alone program if possible, and be functional in Windows 98, 2000 and XP. Access 2002 might function fine, but could anticipate 1,000 to 1,500 patients and that might be too much. SQL might be better, but that aspect is negotiable. Key is data entry-want check-boxes when possible for data forms Would probably prefer to do this is Phases which means if things go well, would be happy to pay the single bid fee which decreases the coders pay-back to RAC. Would in general anticipate the phases something like the following, although certainly adaptable. Or if things are going well, would be happy to negotiate bonus during the completion of the entire project. Phase I. Database and program core, focus on database, data entry and generation of templates and reports Phase II. Interactive functions such as Winfax, Spell check if it needs to be separate, additional "modules" Phase III. Network interactivity such as any separate programs to let assistants interact with the database from a separate computer and enter data, print or fax reports. Possible setup to print standard and routine prescriptions. Also, would like to have the future possible setup to be functional from Tablet or PocketPC. Ultimate goal is to go wireless in the clinic ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. 4) Details of the expected system include: Database tables-related to individual patients: 1) Demographics 2) Problem List 3) Medications 4) Allergies 5) Past Medical History 6) Family History 7) Social History 8) Review of Systems 9) Vital signs 10) Physical exam 11) Other physicians 12) Pharmacy 13) Image storage-anticipate need to store jpg files select X-rays, patient, etc. Additional interactive databases in the system: 1. Physician list 2. Report templates 3. Macro list-insertable phrases/common statements for reports 4. Billing (ICD-9) codes 5. Prescription printing 6. Lab result flow sheet 7. Chemotherapy flow sheet 8. Vitals page with calculations of BSA and creatinine clearance rates Reports concerns: 1. Generated based on the database information as well as categories for HPI, Impression and Plan. 2. Need to be stored and retrievable. 3. Able to be individually archived 4. Exported as RTF or Word document 5. Searchable using filters for date created, printed, etc. 6. Based on Templates that can be stored separately and then called upon to initiate generation of a report 7. Insert capabilities for standard database fields in templates 8. Print capabilities 9. Last note recall and use as a template for subsequent notes Database general functions: 1. Backup function 2. Data entry is key component-desire "check-box" entry into some databases 3. Able to merge database information form same program run on separate computers, or ability to have second worker access same database and input data on a single database on separate computer 4. Import and export of database fields and data 5. Spell check capability 6. Work with Dragon Dictate 7.0 7. Interface with Winfax Pro Further specifics of exact desired layout and specific database fields avaialble. ## Platform Absolute is Windows 2000 and XP Use of Access 2002 or SQL program is negotiable
