Phase 4 of an ongoing web bidding project

In a way this site will have features similar to vWorker, but for homeowners and tradesmen. Currently there are registration forms for the customer and subscriber (tradesman) to join the site. This is a PHP, MySQL and AJAX project. Phases 1, 2 and 3 are done and phase 4 is partially complete. I want to complete the items in red in phase 4. I have included the phase 2 and 3 documents and the green (completed) parts of phase 4 so you get a sense of how it is supposed to work and what has been done. The site URL and login credentials are in the attached phase 4 document. There will be several more phases when this is done, so please only bid if you will be available for future work.
