I need someone to create a survey that I will be emailing to my clients. I want the application to use plain html with CSS (no design is required - I will integrate my design with the code). Here are the steps/pages: 1. User inputs the following criteria: Name (text field), Quality (1-5 star rating), Expertise (1-5 star rating), Responsiveness (1-5 star rating), Comments (text box) 2. User submits the information and if their overall 5 star rating (combine the average of the three 5 star ratings - Quality, Expertise, Responsiveness) is 3 stars or lower they are brought to a Thank You Page. If the user's overall rating is 4 stars or above they are brought to a different thank you page with two buttons (one button for [login to view URL] and another button for [login to view URL]). Their message and overall star rating is displayed on the thank you page. When the user clicks on either button the bottom half of the site (iframe) will display the corresponding website so the user can easily copy/paste their message and post their review.
