CME Attendance and Conference Database PHP/Java mySQL

Online Attendance/Conference management system EASY DB, I know a little about PHP and mysql but by no means able to write a program Attached is a program that is being used right now on a pc(start the program with [login to view URL]) the vision is to do the exact same thing online, with a few more features and a few left out. I have also attached some PDF's with detailed information on how it should be implemented. Web interface could be bare bones; I will add the design to it after. This will need to be a program that can be installed multiple times on the same server. Each program will have to be independent of each other. An install script would be nice but I can just run it from PhpMyAdmin. If a standalone version similar to the program attached can be made a bonus will be issued, or you can respond in your email with a separate bid as to how much it would cost.
