Hi, I would like to have a php/MySQL PETITION SCRIPT
with the following details :
-Text/html petition form and easy to integrate in a webpage
-Form with the records :
*Petition_name (can be put under water – i.e. not visible to visitor)

MySQL-database with the records :

*IP-adres
*Validation (y/n):
*Approval (y/n)
-Validation email – the petition subscriber receives a copy of his submission and a code to validate the subscription to the petition.
-Approval can be set standard as not required and a yes will be posted into the database
-We can also set approval to required in the php-script and ‘we' – the petition manager receive an email to approve the petition-subscriber.
-A confirmation email will be sent to the petition subscriber.
The petition subscibers will be posted into a table (see enclosed word document):

•IP-adresses can be blocked in a list and are unable to post anymore.
•In each mail the IP-adres of the submittor will be integrated.
•I can manipulate the layout easily as well as the font-size and colors of the table.
•A counter which displays the total number of subscribers. I can place / embed the counter every in the site.
•A tell a friend script.
•The form records are easy to extend as well the table and what is displayed.
•The coding has some explanation which variables can be manipulated
•Display an error message when a email is not a real email address
•Display an error message when required records are not filled in. The records records are name and emailadres and a comment can be set to required.
•Separate parts emails, tables, forms can be manipulated very easilly and have a clear structure.
•The form, the counter clock, and the table can be embedded very easily in the webpage in separate spots.
