Personality Assessment Quiz for Website(repost)(repost)

I need a personality assessment quiz script which can be used on my website for lead generation. Here is how the script should function. Front End --------- A visitor comes to the homepage and see the quiz, which comprises of 9 question with 3 multiple choice answers for each question. The user will then select "one" answer for all the nine questions and press the submit button to get the personality type. When the user clicks the submit button he needs to be taken to a page, where he/she can leave their email address and the feedback (along with answers) will be sent to them. Back End -------- How this should actually function is, for every answer the user inputs, there will be a value associated to it. Let's says we term the answers as A/B/C - So the user has to compulsorily select either of the 3 options for each question. At the end of the quiz the system should compute the total number of A's , B's and C's and assign a score. Based on the score the system should send a detailed analysis of the user's personality to their email address. Also, this email address should also be added to the autoresponder that we use. The details of the questions, the answers and the detailed feedback for all the 3 personality types will be given once the project has been accepted. What we need is just the technology. Extras ------ We'll appreciate if the coder can also add some web2.0 jazz to the front end, for e.g. make the design of the webpage web2.0ish and also add simple features like a "evaluating" gif which will be shown after the visitors click the submit button. Any more similar ideas will be greatly appreciated. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform LINUX PHP, Java Script, XML - preffered No ASP strictly, as we have a linux server.
