Build A Simple Web Application

We are looking to hire a programmer to develop a website. Please read the attachment (word doc) for details of the project which also includes a link to a short 5 minute video. The functionality we require is in the word doc and the video - All of this must be included. Essentially it's a PHP application with CSS for design and MYSQL database for data collection. No actual graphical design needs to be added, we will get a html/css designer to add it later - We do want you to add CSS place-holders in the relevant parts of the html to make this an easy task for a designer who does not know php. We are looking to make a start later this week so please bid to be considered.
