I need a form placed on my blog (wordpress). You might integrate a form app on Wordpress that meets project requirements or you may need to create standalone app and then just embed on my Wordpress page. I will provide you with all fields required, and the form should also allow users to upload images and documents. I need this form to be added to my website within 8 hours or less. When a person submits the form in the meantime it should either a be automatically forwarded to an e-mail address or placed into a database from which we can view from the admin area (view data submitted) and download files. I need this complete ASAP, and we can work further on some additional backend pieces for the admin area and managing the content submitted by the user. In addition to bidding, please send me a message via PMB - telling me about your experience, methods of communication, time zone, hour of operation, and any example forms you've created before.
