Health Card

I need a php / mysql project where the user will come to a login page and using the details set up for them by us with a default user and password the user can modifi the password on first login. The user must be able to list all health problems upload a photo. We have a GUI if you want to see the workings of layout in photoshop, xara or pdf. Each user will have his/her own url to let otehrs see the details they want to publish so if they just want name photo and health details with no address they set this from their admin end. This url must never change. so example my url would always be [login to view URL] The user must also be able to save a PDF version of the records they make in their admin end. It would be good if you could use email to get the record as well. Send a mail to a set email address with Ar440E as the subject along with a password in the subject line that the user has set in admin end.. so I send a mail subject Ar440E MyGoodG or what ever I set this will email a reply to the address the mail was sent from of my details agian I can set in the admin end what is sent with the email. If the wrong details are entered then a mail sent back to say wrong details entered. This must also work from mobile phones we have a mobile detect script already that works perfect with 99% of the phones out there. So user from desktop pc sees full site user from mobile is redirected to a mobile version. Email if your intrested in this as I have one more project for both desktop and mobile that needs done as well ASAP. the prson selected will get this project as well as long as we can see a working demos. all source must be open and none encrypted you will be given credits for development.
