Converting JAVA form to ONLINE BASED FORM

We have an electronic version of a form which we fill out when purchasing anything from our walk in clients to have a record of what we bought and from who we bought. Currently we have a JAVA application which makes connection to our DB in one of our servers and stores the information there. With our database growing significantly the JAVA application has become very slow in opening and searching through our records when we need to retrieve customer data. We are looking for an alternate solution which will need to be very quick in terms of using, so perhaps an online based form on our servers as we have a great internet connection so might be a good solution. We do not want any local storage of the database ie on the computers itself for security reasons. I am attaching a few screenshots of the current JAVA form, and the records searching capability and attaching customer Identification documents to the form. We need the system to have a login screen, be able to record customer purchase data according to our requirement, be able to search for customer data using various things such as name, surname, serial number, etc, and finally our exsisting database needs to be merged into the new system so we dont loose any previous data.
