3 Page Website with Database

just to state, you are not to have direct contact with this client. see the attached file. i need help developing my website. it is a fairly simple website with three pages (see sample at [login to view URL]), this site is to be how the pages below will be setup. all data will be provided, but the database needs to be created. [login to view URL] This is the main page for the website. Users will select a location using the drop downs, and select the type of information they want to view for that location (lodging, food, sight-seeing, clubs/entertainment). If the country selected is the US, then drop downs would appear for the state and city. If the location is a non-US location, then only a city drop down appears. Once the last drop down is selected, the web page jumps to the [login to view URL] page and displays the results. [login to view URL] The results page displays the information selected by the user on the [login to view URL] page. The top of the page should remain the same as the [login to view URL] page, the results will display on the bottom of the page. The results page should allow users to add more comments to existing entries, or adding new entries for the location. Users may change the data on the results page by changing their selections on the drop downs at the top of the page. [login to view URL] This page allows users to add more entries to the database for the location selected by the user. The data in the database needs to be readable by search engines (database is a mysql database). See attached for a sample of data, col 1 is state name and col 4 is city information for the US similar file for foreign locations). You would need to define the structure of the database, and load the data, so that it works with the functionality you define for the website. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. They've identified the major cities around world (and I would use their list), but they've also identified an 'all other locations' category. So, if a city is not on my list, users can select 'all other locations' for a country to enter their data. I want to use a predefined list of cities (rather than free text) so that my database doesn't get populated with junk. You wouldn't believe how many difference ways there are to spell Adelaide (Australia).
