83664 Online Quiz

I need an online quiz which will have about 22 questions. Each question is mutiple choice with three possible answers. The possible scores are 1,5,-20. We need to split the 22 questions into 2 groups of 11 and score the 2 results seperatly. Example (-10,35) Once the test is scored there will be 4 outcomes. 1. Both scores are 0 or greater 2. Both scores are less than 0 3. Score A is less than 0 score B is greater than 0 4. Score A is greater than 0 score B is less than 0 An automatic jump to 1 of 4 URLs will result when the quiz is scored. The 22 questions should be broken down into a few pages so that the user does not have to scroll down. Example: 5 questions on the first page and then a please continue (or once all choices are made the next page jumps in). Users should not cheat too easily (using the back button to change answers then go forward again.
