124052 Simple Formmail Modify

I have a formmail script (novice form it's called). I need it modified so that after the user submits the info it will personally thank them by printing to the screen whatever they enter into the name field. So for example if they put Joe123 as there name, after they hit submit it will say on the thank you page: Thanks, Joe123 thats the only modification i need done to the script and I also need the html page i have with the form the way i want it to be made to work with the script. It's just 2 text fields and then a radio button and a submit just make them so they work with the script... The script with the form page the way i want it entitled "[login to view URL]" is attached in a zip file. This job will lead to tons of steady work from me... I can install the script I just need it modified and the page properly done to work with the script...
