1. Home page - load an embedded html file that explain what is this app is about and etc (the html file will be provided) 2. Drug reminder page - for patient to key in all their medications information, i.e.: name, dosage, method of administration, frequency and also the clinic that prescribed it - all medications added will be shown in a list, with a switch allow turning it on and off - they can set a schedule time to remind them to take the medications 3. Self monitoring page - for patient to key in their health parameters, e.g.: blood pressure, blood sugar, weight, exercise log or dietary diary - the saved data can be view in either list or line graph format - the saved data can be exported in PDF format and emailed to the treating physician - the summary should contains patient's identifiers and statistics of all health parameters. - the goal BP and weight should be stated as well - other laboratory parameters, e.g.: hemoglobin, urea, creatinine, sodium, potassium and etc can be added 4. Setting page - allow languages(3) selection (all translated strings will be provided) 5. Forum/chat page - load a forum/online chat page (a ready link will be provided). Proposed GUI will be provided for better understanding of the project.
