minor modifications to a bookmarket script

Please see the bookmarklet available at <[login to view URL]>. IT already shows the source code for a particular selected section in a web page. I want you to slightly re-organise the data displayed, to the format in the xls attached with this message. Each element (text box/radio button/check box/ drop down text box/password box etc) should display one row of data. For eg, please see the sheet named "pressRelease_Login". There are 5 records for form url, form action, form method, form captcha (to indicate if a page has captcha or not) and form_captcha_SRC(to indicate the page which is generating the captcha). And there is one record each for the user id field and the password field in the login page of addpr.com. I need your modified bookmarklet to display data int he format shown in xls. if your bookmarklet can directly write to xls, that would be great, but atleast it should be able to display the data in desired format, so that i can copy it to clipboard and paste it to xls.
