Need website designed with Shopify

I need a talented designer + developer to create a 3 page website using responsive web practices. This will be a very close clone of an existing 3 page website. Please only bid if you can begin this project immediately. I need it completed in 48 hours. This will be a very similar clone of an existing website and you can use the existing HTML. Requirements: 1. The first page of the website needs to have an original header image, an aesthetically pleasing form with 5 fields, and a submit button that sends the data to my email address. 2. The second page will be a buy now page linked to Shopify. 3. The third page is a checkout page. I need the developer to fully upload the live website to my GoDaddy server and have it setup with my Shopify account.
