Design a professional cover and spine for a 1 inch binder

We have 8.5 inches x 11 inch binders that have a clear plastic pocket in the front and on the spine. I would like to print out a professional cover on a standard sheet of paper to put in the front of the binders and also in the 1 inch spine of the binders. Cover should include our logo (InHealth Specialty Pharmacy), address (2345 25th St. S. Fargo, ND 58103) phone number (701) 365-6050, fax number (701) 365-6051, website ([login to view URL]), email (pharmacy@[login to view URL]); pictures of John Deutsch PharmD, RPh and Jason Cossette PharmD, RPh and Heather Novak PharmD, RPh and Elizabeth Payne, PharmD, RPh. Somewhere on the cover should be the words Customized Medication (maybe these two words can be in slightly different sized fonts, with one appearing more bold than the other. Spine should include our logo, and our pharmacy name (InHealth Specialty Pharmacy). These will need to be printed on 8.5 x 11 inch pieces of paper, so the finished product should have 7 or 8 of them, so that we can print them off, cut them out, and then slide into the binders. I included a picture of something to kind of get you thinking about generally what I'm looking for. A professional looking binder. Please feel free to use some different colors that might go well with our logo to give it a custom made appearance. I will send you updated pictures of Heather and Elizabeth as soon as I have those, in the mean time please use the screen shots of outdated pictures I have attached (Heather is the younger looking of the two women) Feel free to be creative, but please make sure it has a professional look to it.
