Excell tracker to record employee performance

need an excel spreadsheet to record monthly employee performance tracking- this would include having about 10 criteria to record employee performance. the results of these performances will decide which top Ten teams get work bonus. It should be an easy format to input new employees in either by manual input or uploading a csv file. I will provide the criteria of which we measure an employees performance
