Data Wizard (Excel wizard needed asap!)

I am looking for an Excel wizard to help me with some Excel data rearrangement (see the attached excel file). Before bidding, make sure that you understand what the assignment is. Also make sure that you will be able to do this assignment. You can even put together a sample from the excel file attachment. This is a very simple project if you are familiar with excel and basic coding. I am only paying after I am satisfied with your work. I am only paying $20 for this project.
