Developer sought to build a tablet/iOS compatible application which uses CLASSIC ASP and VB/VBScript to gather patient details in an emergency situation, and inputs it into a Microsoft Access database. The program must then be able to output a neatly formatted read-only report version of the inforation on command..
The Access database has already been built and is available to review.
We envision the flow of the app as follows:
Login Page - match username and password from database. Basic users can then add new patients. Admin users need to be able to search report forms by Patient details or by Incident Details, as well as adding new patients.
Form 1 - Input Patient Details, check against databaese to prevent duplicates. If no duplicate, move on to Form 2. If duplicate, give user the option to use the existing PatientID or create a new one.
Form 2 - Input Incident Details. Users can then FINALISE if no other pages need to be completed, or add Events using Form 3.
Form 3 - Incident Events. Users can add mutipe events to the Incident, and add detailed comments to each event. Once complete, user can FINALISE and move to Form 4.
Form 4 - Output the whole report in a read-only format with the ability to export to PDF.

The solution must be written in Classic ASP (server side).
****** We have added a requirement for an additional form. This form is used between Form 2 and Form 3 to enter vital signs.
The user should be able to enter multiple sets of vital signs, then when finished, moving on to Form 3 to enter events.
The Access database has been updated to fit.
We apologise for the oversight of missing this from the original RFP.
