Coffee Cup Software- Opt-in WebForm

Looking for someone w/ CoffeeCup software experience. Need someone to use WebForm Builder to create an Opt-in screen to capture email and name of interested visitor to my website. The visitors email and name can be email directly to me and to a autoresponder.

I did some initial work using CoffeeCup and I'm not able to complete this step. Other possible work needed later.
