Broken Autoresponder Needs Fixed

I have a survey/auto-responder that appeared to be working previously. I added some tracking and it seems to be broken now.

I ran ads and tracking software indicated optins, but they didn't show up in Autoresponder.? At that point, I tested in google myself and it worked for me, but didn't (apparently) for other users.

Need to get this fixed and tested.
