Simple Script Clone

Attached file contains a website, I am looking for a clone of the script used on that site for counting votes.

It's a very simple script, all it has to do is to:

- set a cookie for each visitor

- let them cast their vote and register the vote

- prevent from double voting via cookie, and also keep track of what the visitor has voted for, so if they want to try again and vote, they get alerted of what they have voted for before. (check attached sample)

- graphical representation of the votes (horizontal bar)

Also it should be secure enough to prevent drop table or similar attacks.

Admin panel is not necessary. Deliverable would be a sample html page where the script has been implemented, the script itself, all the necessary javascripts, sample database, etc.
