send and print buttons need to be fixed ASAP

I just posted this project earlier today, but the winning bid was unable to complete it,
The website my boss has running has TWO pages that need to be fixed, they are:

and

then "send and print" button on the pages does not function and needs to be fixed ASAP. we need the info on the page to be emailed to this address duncans@mybottleshop(dot)biz
and also include a pop up window with all the information so the clients can print it out.

that works fine, so you can work from that code if you need to,
this job needs to be finished as soon as possible,
so I will give the job to the person who is prepared to complete it right now.
