Create & Design a Wordpress Search Form

I need to create and design a search form for the header of my website - found at [url removed, login to view] (the site is being run with Wordpress). For whatever reason, the code that I was using (attached here as [url removed, login to view]) is no longer working. For this project, I would like the search form to allow for category selection. The form will go in the same spot in the header as it is now.

Attached is a visual of what I am looking for - searchbar.png. The scale, colors and images are only being used as placeholders - whatever can be done, I'll take. I can change the CSS after completion (my current css being used with my form is attached here as [url removed, login to view]).

So, I am requesting a working Wordpress search form code, with category selection, as well as the appropriate CSS file.
