Develop a Microsoft Word add-in application

Need a Microsoft Word 2010 add-in developed in VB.net – only developers experienced in .Net and preferably Word add-ins should bid.
This add-in needs to do the following:
1. User has a document open
2. By selecting the button on the Ribbon UI, user is shown a form which has set of options that they need to choose, as well as a preview of the document
3. Once options form completed and submitted, automatically convert the document to a PDF (easy using existing built-in Microsoft Office Save As PDF functionality)
4. Then email both the PDF and data from the selected options to an email address, plus upload the file and text document of options selected (should be done via FTP easily using [url removed, login to view])
5. Process is complete
This is a simple project, exact details to be used such as options that should be selected, where the email and files should be sent, design of the form etc, will be given to you on awarding this project.
