simple php code for account activations.

I need a simple submit php script that has 3 input fields.

The first where the customer will put their full name.

The second where they put their paypal transaction id.

The last where they put their SIM card number.

once they submit the form it will log their info into a database and email me the info so i can manually activate their info. The same info should not be allowed to be entered more than once.
