Add an interactive map in a contact form (wordpress)

Hi all,
I need a wordpress developer to realize for me an interactive contact form.

I need you to remove the Municipi section and add an interactive map in the form, using the map shown in the attached image [login to view URL]
When user select one of the quarters, they must highlight like in the second image attached, [login to view URL]
If the user goes mouseover on PC, it must appear a window per each quarter of the city showing a text (that will explain the quarters that belong to this zone of the city).
The user must be able to fill all the contact form fields, then he needs to select one or more quarters from the map and then they must be able to submit the form.
I will need to receive all filled content and also a text telling me which quarters of the map have been selected.
