Need [login to view URL] Script that Rewrite Filename Download Based on URL Query String Value

I need a simple script in PHP. This function will force a specified file to download to the users computer using the path to the file to download, as well as the output filename.

Please see this sample site which does the function I need (Ignore the download file)..

For example, there is a file called "[login to view URL]". I want people to download it via [login to view URL] Filename, with a different name (like "New [login to view URL]") and they will see and save "New [login to view URL]" on users computer.

When users click my link <a href="http://path/to/file/[login to view URL]">Download Link</a> the script would get the "[login to view URL]" and instead serve the users a force download to a file "New [login to view URL]". I would like this to work with any filename i put on the download URL parameter http://path/to/file/[login to view URL] FILENAME I PUT HERE BLAH and the script would generate the request on the fly.

I have searched and read some tutorials from the web and I found out that this function will need PHP Header Content Disposition and .htaccess rewrite.

This is a very simple task for anyone who is expert in handing file download using php header content-disposition and dealing with .htaccess rewrite. Please see the above sample website so you'll have a clear idea of what I need to be done.
