Magento custom options design

Hi,
What I need is a custom product page where the design for the custom options is quite special.
I attached a screenshot of how it should look in the end.
The current product page can be seen here: [url removed, login to view]
Special things:
- Select certificate design on the right above the certificate
- Show image next to swarovski star
- Be able to add quantity for swarovski star
- Text area "personal Text" has to be divided into 5 lines with countdown (max 75 characters)
- People should be able to tick a box "foreign language" and then a dropdown should appear where they can select the preferred language.
I already installed the extension [url removed, login to view] which already offers all the things I need.
- Quantity for custom options
- Add radio box to an image so people select the image
- Div styles for custom options
But I can't configure it properly. Documentation of the extension is also attached
There is a problem with the extension (images can't be uploaded as custom options even if it should work)
I can provide a testserver with pre-configured products with all the options I need
