Updating Current Web Lead Form in PHP/Java

At [url removed, login to view] we have a lead form that we recently had a lot of work done on which we were happy with.

However, we are needing to post the lead form to a NEW url (company) which has a different sent of fields and values they need. Some of the form is hidden in PHP and our webmaster simply knows basic java.

1) We need to hire someone to help us change the field data to the new values and make sure the new submission works in the php. The new posting string we have also requires the IP address of the website visitor be passed and the Source value. We'd like this source value to be dynamic and pull from the url string we have going into the site. Meaning if we send someone to [url removed, login to view] it posts the source as "Google" from the url string.

2) Whoever wants to do this project must NOT undo the other php/java work done by the previous programmer. This includes: A) a soap call we do in real-time to check the phone number as they enter it, B) A page that lets the visitor know things are processing before they get to the thank you page and C) we have the description box coded to not allow characters other than standard text. NOTE: These things are finished and working, we just ask that the code isn't broken while updating the form.

3) As we build other sites and update fields and values, we'd love to use the same programmer for future projects as well (If they desire)

I can provide which field data values get change to what once we choose a worker...and the new posting string as well. We also will have a test page set up which can be used before live implementation.
