Create 4 Forms for me

For this project, I want the winner to create 4 html/php forms for me. I will give you a png with exactly what the forms should look like. One of the forms is really, really long. 2 of the forms are sort of long, and 1 form is shorter. Each form should send all of the information to a specified email address in the configuration file you create. You should also allow for multiple email addresses, and different email addresses depending on the form. The short form also requires interaction with paypal since it is a payment form. I will give you the relevant paypal information to complete that part of this project. If you have any additional questions, please PMB me.
