395737 Simple Membership Database

Need to buid a secure free membership site for members to access information in the form of a questionnaire and other docs. The membership component is very simple - just a username and password to log into the site. Once logged in, user should be able to access reports,pdf files,and a 50 questionnaire form and fill in the answers, print the report as often as needed,change answers, share the report with friends and family or just keep private in their account. The website is already built,just need a database backend to allow the creation of online member access to their private data. Site is hosted on a Linux server with Godaddy. Must be built with PHP/Mysql Database. Need done in 5-7 days if possible.
