comibine several html files into 1 dynamic php script

I currently have about 20 html files, all forms leading to 20 separate php mailing scripts. I want to combine all of them into 2 php files, one that process the email and one that serves as the template. Requirements: 1) users right now can go to the addresses as [url removed, login to view] when this goes into effect, I want them to still be able to do that, and the script will know what to show the user 2) depending on the address the user entered, such as [url removed, login to view], I want a different header graphic to be shown, and a word to change on the page so it says "from ___websitename__" 3) depending on address entered, people will be sent an email from someone different, if they go to [url removed, login to view], the email script should email user 1, if [url removed, login to view] then user 2 etc. Here is a zip containing all the current files [url removed, login to view] Thanks Aaron

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):

a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
